Before you create your VicKey account
If you are new to VicKey and have a health questionnaire to complete, you will need to:
- Create an account
- Link a patient profile to your VicKey account for each health service that has requested you complete a health questionnaire
- Verify your identity
What you will need
To create a VicKey account, you will need your own email address.
To link a patient profile and verify your identity, you will need your:
- Health service name
- UR number (patient identification number) for the health service
- Mobile number (matching the number in the patient record for the health service)
